  • NI Number or Recent Tax Document

  • Current DBS Check (If Available)

  • Course Training Certificates

  • Bank Account Details

  • Proof of Address eg: bank statement/utility bill dated within last 3 months

  • Address History for Last 5 Years

  • If you have lived abroad for 3 months or more in the last 5 years, then you will require a police check record from that country.
